Please use this form to document the competitions and scores for the purpose of reporting information necessary to be considered for WNYDA Year End Awards. Send this completed form to the WNYDA Scorekeeper along with copies of the fronts of your tests clearly showing the judges signature and score. These scores will be used for award eligibility and median calculations. See the WNYDA Awards Program details for Award requirements.
You must also send your Required Volunteer Hours form available on our web site or in our Omnibus.
Submissions postmarked after September 30th will not be accepted.
