Required Volunteer Hours Form
 This form
should be used to submit proof of hours volunteered in support of a WNYDA
function and/or for hours volunteered as a WNYDA Board member.
Individuals are required to volunteer for eight (8) hours in order to be eligible for any
WNYDA Award.
